In order to keep it simple. I'll leave it as it is - with the large blank space between the menu and the page content.

I added the values to the answered.
Once I get the validations and error messages figured out, I need to set up the server side to ensure e-mails are handled correctly.
I'll let you know if this works or not.

I recognize there are "tr" elements without a table. I wanted to provide a minimal code set. They wouldn't think they would impact the javascript.

I found a script for cascading dropdown selection lists, i.e., what you select in the first dropdown select determines what choices you will see in the second related dropdown menu.

I'd prefer to not use scripts, but couldn't find another way to tie the field OHV Make to OHV Model.
